Problem Statement

Previous studies in the United States on smoking and diabetes control have not adequately captured the extent of association, particularly for non-Hispanic blacks and women. While cigarette smoking is a well-recognized risk factor for diabetes control, emerging evidence indicates that smokeless tobacco use is also associated with poorer diabetes control, especially among black and Hispanic women. This study used three population-based surveys to assess the extent to which non-Hispanic black, Hispanic, and white women in the U.S. are affected by risk factors for uncontrolled diabetes and other chronic diseases.


Non-Hispanic black women were more likely to smoke (27%) than were non-Hispanic white women (16%), and were more likely to use smokeless tobacco (13% vs. 7%, respectively). Compared to non-Hispanic white women, non-Hispanic black women had higher body mass index (35.0 kg/m2 vs. 32.0 kg/m2, respectively), were more likely to have diabetes (26% vs. 20%, respectively), and were less likely to use antidiabetic medications (59% vs. 65%, respectively). Further adjustment for cigarette smoking and smokeless tobacco use increased the relative risk of having uncontrolled diabetes (RR = 1.53) for non-Hispanic black women (and not for non-Hispanic white women), but had little impact on relative risks for uncontrolled diabetes (RR = 1.05) or medication use (RR = 0.93). Smoking and smokeless tobacco use were similarly associated with medication use in non-Hispanic black and white women. In general, black and Hispanic women had a lower prevalence
